Revocation of Power of Attorney
If the donor (the person granting a power of lawyer) decides to revoke the document, it is almost always required that the revocation be in writing. This form is to be sent by the donor to the lawyer informing them that the power is revoked and confirming all actions previously taken by the lawyer.
