Alberta Personal Directive Package
If you're an Alberta resident, you can make a Personal Directive with this easy-to-use package.
A Personal Directive ensures that your wishes are followed in the event that you become physically or mentally incapacitated. It is also called an advance health care directive or a living will.With a Personal Directive, you can appoint an agent - someone you trust - to act on your behalf, and you can give your agent detailed instructions and set out your wishes regarding medical care, living conditions, personal and legal matters.
