General Affidavit for Two Persons
A General Affidavit is used to make certain statements in written form under oath. This General Affidavit form requires that the Declarant, that is the person signing the affidavit and making the declaration, to state that certain facts set forth in the Affidavit are true and correct and are sworn to in the presence of a notary. This particular variation of the affidavit form is designed for use by two declarants.
