Pet Information Form for Pet Sitters

Pet sitters, have your clients fill out this Pet Information Form for each pet that you are looking after.

The pet owner must fill in all the information you will need to know regarding:

• physical description, character, personality, likes and dislikes, favorite toys, sleeping spots, etc.,
• food, feeding schedule, special dietary needs,
• house training issues, toilet habits, behavior issues,
• medical history, health problems, medication, vet information,
• location of food, medicines, cleaning supplies, leashes, etc.,
• social behavior of animal, events that spark fear or anxiety, etc.
